Membership Info.
All new swimmers with Caradon Swimming Club must complete an A.S.A Membership form.
Swimmers who are taking regular medication (Inhalers) need to complete an A.S.A Medical Declaration Form (Annually) and Club Medical Forms on joining. Both forms should be given to you on joining the club, if not then they are available from any committee member. The method of payment is 1st Monthly Standing Order
Re-issuing of standing order forms will given to you when you move squads. If fees fall into arrears of go unpaid the Club reserves of right to withdraw membership.
